Team Registration

This form is for players who are registering their entire team at one time. Team registration is $400. With registration, each team is entitled to 4 t-shirts. Be sure to check shirt sizes under each name when listing players below. Alternate players can purchase shirts on the day of the event. 4 players must compete at all times to qualify for matches, so, list 2 alternate players if you think your primary players will not be available for the entire day. All payments are made securely using PayPal.
